From intake to follow-up.

  1. 01

    You fill out an intake

    Before booking, every patient completes a structured clinical intake. The intake covers exposure history, travel and residence, household contacts, occupational and environmental factors, prior workup, current medications, and current symptoms. The intake is reviewed by the clinician before the appointment so consultation time is spent on clinical reasoning, not on data collection.

  2. 02

    You book and pay

    Patients book a video consultation through the patient portal. Payment is collected at booking. The standard appointment is $120 for an individual and $100 for a household member booking under the same primary patient.

  3. 03

    You meet your clinician on video

    The consultation is delivered by a nurse practitioner or physician assistant practicing under standardized procedures and a delegation of services agreement with the supervising physician. The clinician reviews the intake, gathers any additional history needed, and develops an evaluation plan.

  4. 04

    Tests, if you need them

    When testing is indicated, orders are placed through the EMR. Lab work is performed through Quest Diagnostics or LabCorp, depending on the panel and the patient's location. Imaging, when relevant, is referred to a partner imaging center. The patient is billed for labs and imaging directly by the performing provider.

  5. 05

    A prescription, if you need one

    When pharmacotherapy is indicated, the prescription is sent electronically to the patient's pharmacy of choice. Nurse practitioners and physician assistants on this service have prescriptive authority for the medications relevant to parasitology practice. Patients pay the pharmacy directly.

  6. 06

    Follow-up, if it's recommended

    Follow-up appointments are scheduled when clinically warranted, for example to review test results, adjust treatment, or monitor response. Asynchronous messaging through the patient portal is available between appointments for clinical questions about the active workup.

Electronic medical record and patient portal

ChARM Health

ChARM Health is the EMR of record. It hosts the patient portal, intake forms, scheduling, video consultations, electronic prescribing, lab integrations, and billing. ChARM is HIPAA-compliant and operates under a Business Associate Agreement.

Lab partners

Quest Diagnostics and LabCorp

Lab orders are routed to Quest Diagnostics or LabCorp depending on the panel and the patient's location. Patients receive draw orders and use the partner's network of patient service centers.

Supervising physician

Dr. Waltzman, MD

Dr. Waltzman holds California licensure and supervises clinicians on this service under California regulation. The supervising physician sets clinical protocols, reviews cases as required, and is the ultimate clinical authority.

A consultation is $120.

Flat, private pay, no insurance. Labs, imaging, and prescriptions are billed separately by the lab, imaging center, and pharmacy.
